USCIS Form N-426: Request for Certification of Military or Naval Service

Until then, you can also use the 03/09/23 edition. You can find the edition date at the bottom of the page on the form and instructions.

Where to file

Form N-426 should be filed or uploaded with the electronic Form N-400, Application for Naturalization, or if submitting by paper, using the Direct Filing Addresses for Form N-400, Application for Naturalization. If you received a Request for Evidence asking you to submit a Form N-426, return the Form N-426 to the office that requested the certification.

If you are claiming eligibility for naturalization based on your service in the U.S. armed forces and you are currently serving, you must submit a signed and certified Form N-426 with your Form N-400, Application for Naturalization. Only authorized military personnel may certify your Form N-426.

Please review your branch’s policy regarding who is authorized to certify. If you previously served in the U.S. armed forces, do not submit Form N-426.

Instead, submit a copy of your DD Form 214, Certificate of Release or Discharge from Active Duty; NGB Form 22, National Guard Report of Separation and Record of Service; or an equivalent discharge document for all periods of service.
