Initial evidence

Did you provide the following? A personal statement; Evidence you are or have been a victim of a severe form of trafficking in persons; Evidence you are physically present in the United States, American Samoa, Commonwealth of the Northern Mariana Islands, or at a port of entry, because you were trafficked; Evidence you have complied with any reasonable request for assistance in a law enforcement investigation or prosecution of acts of trafficking, unless you were under 18 years of age at the time of victimization or you are unable to cooperate with a request due to physical or psychological trauma; Evidence you would suffer extreme hardship involving unusual and severe harm upon removal; If you are inadmissible, an application for a waiver of inadmissibility on Form I-192; Credible documentation of the claimed relationship if you are filing Form I-914, Supplement A, for an eligible family member;
