USCIS Form I-693: Report of Immigration Medical Examination and Vaccination Record

Edition date

01/20/25. If the civil surgeon signs your form on or before July 2, 2025, you must use the 03/09/23 or the 01/20/25 edition.

If the civil surgeon signs your form July 3, 2025 or later, USCIS will accept only the 01/20/25 edition. You can find the edition date at the bottom of the page on the form and instructions.

Where to file

After completing your immigration medical examination, the civil surgeon must give you the completed and signed Form I-693 in a sealed envelope. Do not accept the Form I-693 if it is not in a sealed envelope.

You (not the civil surgeon) must submit your Form I-693 to USCIS. Submitting Your Form I-693 If You File Your Adjustment of Status Application With USCIS by Mail If you are applying for adjustment of status to lawful permanent resident (a Green Card) by mail, you must submit Form I-693 by mail with your Form I-485, to the location specified for your Form I-485 (see the Direct Filing Addresses for Form I-485).

Otherwise, we may reject your Form I-485. Submitting Your Form I-693 If You File Your Adjustment of Status Application With USCIS Online If you file your Form I-485 online, you must open the sealed envelope from the civil surgeon and upload the completed Form I-693 with your Form I-485 application package.

You must keep the original Form I-693 and the envelope from the civil surgeon until we make a final decision on your Form I-485.

When to file

If you are filing Form I-485, and if you are required to submit a Form I-693 or partial Form I-693 (such as the Vaccination Record), you must submit it with your Form I-485. Otherwise, we may reject your Form I-485.

Special instructions

eMedical The Centers for Disease Control and Prevention (CDC) updated the Tuberculosis Technical Instructions for Civil Surgeons. Civil surgeons must register for access to eMedical, an electronic health processing system.

Using eMedical will help the CDC maintain public health security and transition to electronic submission of immigration medical examinations. Civil surgeons should have received emailed instructions to register; it is important that you follow these instructions and register immediately.
